Minor Body Piercing Requirements
- Parent must be present
- State issued identification for the minor
- (Learner’s permit, state issued ID, passport, or birth certificate are acceptable. Student IDs will not be accepted as an appropriate form of ID)
- State issued identification for the parent
- (State issued ID, driver’s license, or passport)
- Notarized consent form
- (We DO NOT have a notary on site. These documents need to be notarized prior to coming in for your appointment. Notaries can be completed at your local bank or at Amscot)
