First session for couples. Two forms need to be signed by both partners: Intake form and CC Authorization Form.

Initial family session with Rising Calm Center. All parties (adults and children) must sign the intake form in order to receive services.

For existing clients - additional sessions with Rising Calm Center.

Sessions are Rising Calm Center office. (4421 NW 39th Ave., Building 3, Gainesville, FL 32606)
