Uric acid measurements are useful in the diagnosis and treatment of gout, renal failure, and a variety of other disorders including leukemia, psoriasis, starvation, and other wasting conditions. Patients receiving cytotoxic drugs may be monitored with uric acid measurements.

Semiquantitative culture to isolate and identify bacterial causes of urinary tract infection.

Diagnose pancreatitis, more specific for pancreatitis than is serum amylase; diagnose peritonitis, strangulated or infarcted bowel, pancreatic cyst.

The primary clinical use of LH measurement is in evaluating the normalcy of hypothalamic-pituitary-gonadal axis. Measurement of serum gonadotropin levels will allow for distinguishing between primary gonadal failure and deficient gonadal stimulation. LH measurement may also be of clinical importance because growth hormone and LH are frequently the first hormones to be affected by pituitary disease. The serum analysis of LH has also been found to be very useful in the diagnosis and treatment of infertility in women.

This insulin assay is performed using a 2-site electrochemiluminescent immunoassay on the Roche automated platform.1 This test could be effectively used for those patients not receiving exogenous insulin or those receiving unmodified human insulin. Test 004333, Insulin is highly specific for human insulin, as shown in the table below. This test does not react, for instance, with several of the insulin analogs used to treat diabetes as indicated in Table 1 below. The efficacy of each test depends on the patient treatment and goal of the ordering physician.

Dihydrotestosterone and dihydrotestosterone/ratio may be used for differential diagnosis of diseases of sexual differentiation (DSD). DHT levels may be inappropriately low compared to testosterone levels in 5a-reductase deficiency. It is recommended by guidelines to measure the ratio of testosterone to DHT.

Prostate-specific antigen (PSA) is a glycoprotein produced by the epithelial cells lining the prostatic ducts and acini. Normally, it is secreted into the prostatic ducts and is present only in prostate tissue, prostatic fluid, and seminal plasma. PSA is produced by normal, hyperplastic, and cancerous prostatic tissue. PSA is used as a tumor marker for the early detection of prostate cancer and in other areas of prostate disease management.

Diagnose VZV infection; determine adult susceptibility to infection.

Rule out vitamin D deficiency.

Valproate (valproic acid; divalproex sodium, a compound containing sodium valproate and valproic acid) controls absence, myoclonic, and tonic-clonic seizures in generalized, idiopathic, and symptomatic epilepsy. It is most useful in typical absence seizures. Valproate is as effective as ethosuximide in patients with absence seizures alone and is variably effective in atypical absence seizures.

This test is indicated for use as an aid to the diagnosis of West Nile virus encephalitis.

Presumptive qualitative detection of Zika virus IgM antibodies in human sera collected from individuals meeting CDC Zika virus clinical criteria (eg, a history of clinical signs and symptoms associated with Zika virus infection) and/or CDC Zika virus epidemiological criteria (eg, history of residence in or travel to a geographic region with active Zika transmission at the time of travel, or other epidemiological criteria for which Zika virus testing may be indicated).

This test can be used to follow treatment response in patients being treated for syphilis infection. This assay should not be used to screen for syphilis infection.

Evaluation of the extrinsic coagulation system; aid in screening for congenital and acquired deficiencies of factors II, V, VII, X, and fibrinogen.6-8 This test is used clinically for the therapeutic monitoring of warfarin (Coumadin®) anticoagulant therapy.

A phosphate in blood test may be used to diagnose and monitor kidney disease, bone disorders and diagnose parathyroid disorders. Parathyroid glands are small glands located in the neck. They make hormones that control the amount of calcium in the blood. If the gland makes too much or too little of these hormones, it can cause serious health problems.

Kidney function tests are common lab tests used to evaluate how well the kidneys are working.

Evaluate erythropoietic activity which is increased in acute and chronic hemorrhage, and hemolytic anemias; evaluate erythropoietic response to antianemic therapy.

The insulin-like growth factor (IGF) system plays a critical role in the growth and differentiation of normal and malignant cells. The components of the IGF system include growth hormone (GH), IGF-I and II, type I and II receptors, IGF binding proteins and proteases. IGF binding proteins were first identified for their high affinity interactions with IGF-I and IGF-II.2

IGFBP-3 is the most abundant IGFBP species in circulation and binds 75% to 90% of circulating IGF-I in a ternary complex consisting of IGFBP-3, IGF-I and acid-labile subunit (ALS). IGFBP-3 modulates the activity of IGF-1 and to increase their half-lives. It has been postulated that IGFBP-3 is regulated by GH and originates in the liver as low levels were observed in patients with impaired hepatic function.

Tacrolimus is an immunosuppressive drug that is believed to prevent rejection in transplantation patients. Measurement of tacrolimus blood levels may be of use in monitoring patients receiving this drug.

The Estradiol (E2) Blood Test measures estradiol levels, one of the primary components of estrogens, in the blood.

Quantitative HBV surface antigen (HBsAg) testing is intended for use in individuals with a confirmed diagnosis of Hepatitis B Virus infection based on positive HBsAg, Anti-HBs antibody and/or Anti-core antigen (anti-HBc) antibody test results. Quantitative HBsAg testing has utility in assessing HBV replication in the absence and presence of antiviral therapy, which may inform monitoring treatment response and relapse in the setting of initial and prolonged antiviral therapy, respectively. Recent studies indicate that rapid decay and loss of HBsAg expression are strong predictors of sustained HBV clearance.

This test is done to detect possible IgE-mediated responses to various foods. Food items tested are: chocolate-cacao, corn, egg (whole), cod fish. Shrimp, salmon, beef Tuna, Mussel, milk (cow), peanut, pork, soybean, and wheat.

An AFP tumor marker test may be used to: Help confirm or rule out a diagnosis of liver cancer or cancer of the ovaries or testicles.

Monitor the health of people with cirrhosis or hepatitis.

Evaluate adrenal carcinomas that frequently secrete large amounts of DHEA.

This assay is intended for the in vitro quantitative determination of carcinoembryonic antigen in human serum and plasma.1 The Elecsys CEA assay is further indicated for serial measurement of CEA to aid in the management of cancer patients. The main indication for CEA determinations is the follow-up and therapy-management of colorectal carcinoma.

A cortisol test is used to help diagnose disorders of the adrenal gland. These include Cushing's syndrome, a condition that causes your body to make too much cortisol, and Addison disease, a condition in which your body doesn't make enough cort

Detection of gliadin antibodies aids in the diagnosis and monitoring of certain gluten-sensitive enteropathies, such as celiac disease and dermatitis herpetiformis.

Evaluate postmenopausal vaginal bleeding due to peripheral conversion of androgenic steroids. Increased estrone levels may be associated with increased levels of circulating androgens and their subsequent peripheral conversion.

Diagnose diabetes mellitus; evaluate disorders of carbohydrate metabolism including alcoholism; evaluate acidosis and ketoacidosis; evaluate dehydration, coma, hypoglycemia of insulinoma, neuroglycopenia. A fasting glucose ≥125 mg/dL on more than one occasion is adequate for the diagnosis of diabetes mellitus. An OGTT is not necessary in this setting. Infants, especially with tremor, cyanosis, convulsions, and respiratory distress should have stat glucose, particularly if there is maternal diabetes, postmaturity, asphyxia, hemolytic disease of the newborn, or possible sepsis. Babies too large or small for gestational age should also have glucose in the first 24 hours of life. Random blood sugars can be used to monitor therapy in diabetics or evaluate presence of insulinoma..

Hemoglobin A1c values are used to assess glucose control in diabetes, and in 2010 the American Diabetes Associations affirmed the decision of an international expert committee recommendation to use the A1c test to diagnose diabetes with a threshold ≥6.5%. Point-of-care A1c assays are not sufficiently accurate at this time to use for diagnostic purposes.

Qualitative determination of presence of hemoglobin S; detect sickling hemoglobins; evaluate hemolytic anemia, undiagnosed hereditary anemia with morphologic (sickle-like) abnormalities on peripheral blood smear.

Liver function tests (also known as a liver panel) are blood tests that measure different enzymes, proteins, and other substances made by the liver. These tests check the overall health of your liver.

This assay is intended for use as an aid in the diagnosis of acute or recent infection (usually 6 months or less) with hepatitis A virus (HAV). This is an in vitro diagnostic immunoassay for the qualitative determination of IgM response to HAV. The presence of IgM antibody to HAV is diagnostic of acute HAV infection.

This assay may be used in combination with other hepatitis B virus (HBV) marker assays to define the clinical status of known HBV infected patients or can be combined with other HBV, HAV (hepatitis A virus), and HCV (hepatitis C virus) assays for the diagnosis of patients presenting with symptoms of acute viral hepatitis.

Qualitative detection of antibodies to HCV. Per current guidelines, this test should not be used alone to screen for and diagnose HCV infection. HCV antibody positive patients should be tested for HCV RNA to differentiate between previous and active infection.

Screen patients who may be at risk for heart disease and stroke.

A principal use of this test is in the detection of monoclonal gammopathies. These are usually found in association with hemic neoplasms, especially multiple myeloma. They also occur in other benign and malignant conditions. Any such protein detected should be identified by an alternative technique, such as immunofixation or immunoelectrophoretic.

Phenytoin is useful in generalized tonic-clonic, complex partial, and simple partial seizures and frequently is chosen for initial therapy, particularly in adults. Because of phenytoin's potential adverse reactions (hirsutism, gingival hyperplasia, coarsening of facial features), other antiepileptic drugs are often prescribed for infants and young children.

A Carbamazepine Blood Test is a test that evaluates the blood levels of carbamazepine. It is used in the dose adjustment and to ensure that carbamazepine levels remain within an acceptable range.

Help in the differential diagnosis and prognosis of arthritic disorders.

Determine Rh type for transfusion candidates. Rh typing of expectant mothers may indicate potential for Rh hemolytic disease of the fetus/newborn. This typing is also used to determine Rh immune globulin candidacy for prenatal and postpartum patients.

Evaluate the nonspecific activity of infections, inflammatory states, autoimmune disorders, and plasma cell dyscrasias.

Levels of SHBG are under the positive control of estrogens and thyroid hormones, and are suppressed by androgens. These influences dynamically control the liver synthesis of this carrier protein.

Aids in the diagnosis of Sjögren's syndrome (especially with vasculitis) and lupus patients with Sjögren's overlap syndrome.

Assess immunity against tetanus by determining levels of circulating antibodies to tetanus toxin or to measure the immune response, post-vaccination, in individuals suspected of immunodeficiency disorders.

Creatine levels may be significantly increased in amyotrophic lateral sclerosis, dermatomyositis, myasthenia gravis, starvation, muscular dystrophies, and trauma. Creatine synthesis is stimulated by methyltestosterone and may also be increased in hyperthyroidism, diabetic acidosis, and puerperium.

Albumin blood test evaluates nutritional status, blood oncotic pressure, evaluation of renal disease with proteinuria, and other chronic diseases.

Thyroid function test. Investigation of low thyroxine (T4) result; the differential diagnosis of primary hypothyroidism from normal, and the differential diagnosis of primary hypothyroidism from pituitary/hypothalamic hypothyroidism. TSH is high in primary hypothyroidism. Low TSH occurs in hyperthyroidism. Evaluation of therapy in hypothyroid patients receiving various thyroid hormone preparations: Low values are found in states of excessive thyroid replacement. Normal result on a sensitive TSH assay is acceptable evidence of adequate thyroid replacement.

Excessive FSH and LH are found in hypogonadism, anorchia, gonadal failure,1 complete testicular feminization syndrome, menopause, Klinefelter syndrome, alcoholism, and castration. FSH and LH are pituitary products, useful to distinguish primary gonadal failure from secondary (hypothalamic/pituitary) causes of gonadal failure, menstrual disturbances, and amenorrhea. Useful in defining menstrual cycle phases in infertility evaluation of women and testicular dysfunction in men. FSH is commonly used with LH, which also is a gonadotropin. Both are low in pituitary or hypothalamic failure. FSH and LH levels are high following menopause.
